On March 11, 2018, a sightseeing helicopter lost power and came down in the East River in New York City. The pilot survived, but all five passengers drowned. It’s a tragic example of how multiple factors come together to produce a disaster. The webinar shows each slice in the Swiss Cheese accident model.
